Diabetes Coalition of Santa Clara County

Our Mission

The mission of the Diabetes Coalition of Santa Clara County is to improve the health and well-being of low income, underserved people at risk for, or living with, diabetes by implementing the chronic care model throughout the system of care.

Objectives

To ensure that the service delivery system in Santa Clara County, and the organizations within it, have the organizational abilities to provide care that meets the needs of low-income and underserved people living with diabetes.
To develop and implement a comprehensive strategy to standardize the system of care around diabetes for low-income, underserved populations in Santa Clara County.
To engage a wide range of policy and decision-makers around the local and broader systems changes necessary to fully implement the chronic care model in Santa Clara County.
To mobilize community resources to provide patients and their families with the knowledge, skills, and support to take responsibility for managing their diabetes and their health.
To maintain a vibrant coalition, involving the wide range of providers and perspectives necessary to provide the long-term leadership to transform the system of diabetes care in Santa Clara County.

What's New

Most Summer Camps are Full!

Registration for 2010 summer camps opened on 1 March 2010 and, as of July 12, all but our Sacramento Day Camp are already full. Full payments or payment plan requests were due 1 April. Financial aid (campership) requests were due 15 April (for round 1 funding). The Lake Tahoe Weekend Family Camp and San Jose Day Camp and Camp De Los Ninos, the Counselor-in-Training program, Camp Kaiser, Teen Camp DJ, and Johnson Youth Camp are now full. We are happy to say that there are still openings for the Rosenberg Sacramento Day Camp on 9/25/2010; please fill out and return a 2010 registration form if you are interested .

We are no longer adding names to the waiting lists for full programs, as the existing lists are longer than we can reasonably expect to fill.

Registered campers and families can find camps packing lists, transportation information, directions, and insulin dosage cards on our website's "forms" page, referenced from the grey side menu on your left as "Downloadable Forms".

Camp Kaiser

The Diabetes Society in partnership with Kaiser Permanente Hospital is sponsoring Camp Kaiser for teens 10 to 17. Campers must be members of Kaiser Hospital in Sacramento.

Diabetes Friendly Cooking Class

Practical skills in meal preparation for diabetics & pre-diabetics. How to stock a diabetic-friendly pantry and refrigerator. Learn efficient cooking techniques & ingredient substitution, with emphasis on how to make one creative preparation stretch across several meals.
