Countywide Chronic Care Coalition - C4
Our Mission
The mission of the Countywide Chronic Care Coalition is to promote adoption of the Chronic Care Model of health care throughout Santa Clara County. Adoption of the Model will improve the effectiveness and efficiency of care, and will increase access for patients to needed care.
Objectives
The Coalition will focus on three central components of the Model:
- Provide appropriate care in the medical office and elsewhere.
- Assure opportunities for patients to learn about their conditions and their treatment, and about ways they can help manage them (self-management).
- Share of clinical information among health care providers across the county to achieve better continuity and integration of care.
